Certification Application Process
The following is a step-by-step guide to certification candidates to complete the application process for CMSL certification.
Step 1: Complete the application formYou will find a link to download the application for CMSL certification at the bottom of this page. Complete the form and hit "submit" to open the file, and follow the instructions. You can fill in the file in electronic format and save it to your desktop, or print the application out and fill in the hardcopy. To begin the application for CMSL certification candidates should
Option 1: Download and print the Candidate Application Form (available for download in PDF format), complete it and mail the application to:

Option 2: Complete the application online by filling in each field on the application and submit electronically to: application@msleadercertification.com.
Step 2: Submit the application feeIn order to commence processing the candidate's application, the application fee must be received. This can be submitted by check or money order with your application, or to pay by credit card, call the program toll free at 800-951-9403.
The fee for the certification application process and examination is $395 and is non-refundable.
If the candidate's primary hospital affiliation is with a hospital that is a member of the Greeley Medical Staff Institute, the application fee is $325. The fee may be paid by credit card, personal check, or money order for the total amount, payable to HCPro, Inc. "Medical Staff Leadership Certification." Checks drawn on non-United States banks must state "Payable in US Dollars". Please be sure the candidate's name is written on the face of the check. An additional charge of $35 will be added for any returned checks or rejected credit cards.
Step 3: Demonstrate Completion of the Required CurriculumTo demonstrate that the applicant has completed the required curriculum, applicants should
Option 1: Download and print and complete the Required Curriculum Form (available for download in PDF format). The candidate must submit the completed Required Curriculum Form, along with a copy of the timed agenda and CME certificate for each course the candidate is applying to the curriculum requirements. These materials must be mailed to HCPro, Inc, Attention: Certified Medical Staff Leader, 200 Hoods Lane, Marblehead, MA 01945. In some cases, additional course materials may be requested by the program administrators to confirm the required content has been adequately covered in the program.
Option 2: Complete the Required Curriculum Form online by filling in each field on the application and submit electronically to: application@msleadercertification.com.
To be eligible to sit for the certification examination, a candidate must be a physician, dentist or podiatrist in good standing on a medical staff who has completed the mandatory medical staff leadership training curriculum. For detail on the subject matter to be covered under each of these curriculum areas, please visit the Program Requirements page on this site or review the Candidate Handbook.
Step 4: Program Assessment of Curriculum CompletionUpon receipt of the completed application and supporting materials, the program will determine if the required curriculum areas have been adequately completed. If a positive determination is made, the candidate will receive instructions for taking the certification examination online. If a determination is made that one or more of the required areas has not been adequately covered in the candidate's educational experiences to meet certification eligibility requirements, the candidate will be so informed.
Please note that the Medical Staff Leadership Certification Program reserves the sole right to determine which educational experiences meet the requirements for the mandatory training. Candidates who are determined not to have met all of the required training may submit evidence of additional training at a later date without having to initiate a new application process, provided the additional training is completed within five years of submission of the initial application.
Step 5: Complete the Certification ExaminationInstructions for completing the certification examination will be sent electronically to the applicant. The applicant may take the examination at any time after the examination becomes available January 1, 2008. The examination is an "open book" examination, because medical staff leaders carry out their responsibilities with access to numerous informational resources.
